It would be wonderful if patients were in their best physical condition, well nourished and off medications, particularly steroids, before the operation. But surgery is usually performed when someone isn't doing well, so most of those with Crohn's disease or ulcerative colitis are on medicines and aren't in their best shape or eating well when they need surgery.

The gastroenterologist or surgeon may prescribe extra nutritional drinks or provide nutrition though a vein (intravenously, I.V.) beforehand and afterwards as well. Most of the time, the surgeon or the surgeon's office will provide specific instructions about the diet, when to stop eating before the procedure and when it's safe to eat again. 

"Stress doses" of steroids may be needed at the time of operation and for a short while afterwards, if you have been on steroids within the previous year. Antibiotics may be started as well to control or prevent an infection.  

Additionally, there's an emotional roller coaster that many families feel when surgery is recommended or considered. The important point is that the person undergoing the surgery needs to know that support and extra nurturing is being offered freely and care.
